What is a Support Plan?

 

This is a record of what you agree to do in order to help you reach the aims you have said you have. Your keyworker will support you in agreeing the most appropriate things you need to work on and may be as large or as small as required. Each plan is geared to the individual person. At the end of each 3 month period, your plan will be reviewed and you will have the opportunity to see how you have progressed.

 

Below is a screen shot of the software used (known as Provider), to record support plans and objectives. You will be given the opportunity to influence and agree which aims and objectives are appropriate for you as an individual to help you progress with your life goals. The example below shows an objective has been set that helps secure that persons accommodation by ensuring that their rent is paid in a timely manner - crucial if you want to stay at the scheme.
